Scheduling
Drug testing– We are set up on a walk-in basis for all drug testing and drug screen collections. If you received information from your potential employer that states you must make an appointment- please disregard this. We will not schedule an appointment for you. Our hours of operation are 8am-4:30pm Monday thru Friday. Bring any paperwork including referral forms and a valid photo ID. You must have a valid, government issued or school photo ID. A paper ID from the BMV will work as a valid ID.
TB Skin Testing/PPD/Mantoux Testing- We are set up on a walk-in basis for all TB skin testing. If you received information from your potential employer that states you must make an appointment- please disregard this. We will not schedule an appointment for you. A TB skin test may only be done Monday-Wednesday or Fridays. We are not open on the weekends to read tests placed on Thursdays. You must have a valid, government issued or school photo ID. A paper ID from the BMV will work as a valid ID.
DNA/Paternity Testing- We are set up on a walk-in basis for all DNA/Paternity Testing. We do ask that you arrive at the clinic no later than 4:00pm for a DNA collection. Mother and alleged father need to provide valid, government issued or school photo ID. It is helpful but
not necessary to bring a birth certificate or birth confirmation letter for the child. If your child is breast fed, please be sure he/she has not eaten within 2 hour of arrival for collection. Breast milk can affect a DNA outcome.

Blood Lab Services- We can accommodate either walk-in or scheduled appointments. If you need a fasting lab drawn and want to be seen early morning, please call us at 765-447-8700 x5 or toll free at 877-902-9225 x5 to schedule. We are very busy most mornings and want to be sure you have the shortest wait time possible. If you walk-in in the morning, you may experience a longer wait time. If you or your physician would like to fax your orders to us this will also speed up the process. Our fax number is 765-447-8701.
Wellness/Biometric Testing- We can accommodate either walk-in or scheduled appointments. You will need to be fasting for this testing to get your most accurate results. If you want to be seen early morning, please call us at 765-447-8700 x5 or toll free at 877-902-9225 x5 to schedule. We are very busy most mornings and want to be sure you have the shortest wait time possible. If you walk-in in the morning, you may experience a longer wait time.
